Болезнь Бехчета и ассоциации с антигеном НLА-В5 (обзор литературы и собственные данные)

Аннотация

Цель исследования. Оценить распределение антигенов НLА класса I (А, В) у больных болезнью Бехчета (ББ) и связь антигена В5 с клиническими проявлениями заболевания в различных этнических и популяционных группах в зависимости от половой принадлежности.
Материалы и методы. В исследование включили 93 больных (68 мужчин, 25 женщин) из представителей 24 национальностей с достоверной ББ. Типирование антигенов НLА класса I осуществляли стандартным микролимфоцитотоксическим методом с использованием набора антилейкоцитарных сывороток ("ГИСАНС", Санкт-Петербург).
Результаты. Распространенность антигена HLA-В5 у больных ББ оказалась достоверно выше, чем в контроле (72,0 и 21% соответственно) и была сходной у больных разных народностей, проживающих на территории Кавказа и Закавказья (80-83%), в то же время число больных ББ, позитивных по антигену HLA-В5, в русской популяции было в 3 раза меньше, чем у других больных ББ (р < 0,01). Выявлена достоверная корреляция антигена HLA-В5 с поражением глаз (ангиит сетчатки), преимущественно у больных ББ мужского пола.
Заключение. Распространенность антигена HLA-В5 у больных ББ достоверно выше, чем в популяционном контроле. Диагностическая значимость этого антигена, например, в русской популяции больных ББ не столь велика. Наличие антигена HLA-В5 в фенотипе больных ББ мужского пола можно рассматривать как прогностически неблагоприятный маркер развития патологии глаз.
