Incidence rate of celiac disease in patients withchronic diarrhea
Abstract
Aim. To study prevalence of celiac disease (CD) among patients with chronic diarrhea (ChD).
Material and methods. Serum levels of IgA-antibodies to gliadin, endomysium, reticulin and tissue
transglutaminase were examined in 206 patients with CD. Biopsies were obtained from a distal portion
of the duodenum or a proximal portion of the jejunum.
Results. CD was diagnosed in 35 (16.9%) of 206 patients with ChD. The disease was typical in 5
(2.4%) patients and was latent in 30 (14.5%) patients. Antibodies to gliadin, endomysium, reticulin
and tissues transglutaminase in diagnostically significant titers were detected in the serum of all the examinees
with CD. Formation of the antitissue antibodies occurred because of destructive-dystrophic alterations
of the connective tissue of the lamina propria mucosae pointing to the autoimmune nature of
a pathological process in celiac disease.
Conclusion. To diagnose CD in ChD patients, it is necessary to supplement standard examination with
duodenobiopsy, tests for antibodies to gliadin, tissue transglutaminase and to tissue structures - endomysium
and reticulin.

Effects of p-blockers metoprolol CR/XL andcarvedilol on left ventricular contractility in patients withchronic cardiac failure
Abstract
Aim. To compare effects of metoproloi CR/XL and carvedilol on global and regional contractility of
the left ventricle in patients with chronic cardiac failure (CCF).
Material and methods. The results of two trials were compared: the effects of metoproloi CR/XL and
on carvedilol left ventricular contractility in patients with chronic cardiac failure (CCF). A total of 52
patients with ischemic heart disease (IHD) and dilated cardiomyopathy (DCMP) entered the studies.
Of them, the treatment was completed in 19 and 20 patients, respectively. Ejection fraction (EF), left
ventricular end systolic and diastolic volumes were measured at radionuclide ventriculography before
and 6 months after the start of the treatment.
Results. Both ^-blockers significantly increased EF and reduced the volumes. In both groups the rise
of contractility in EF < 30% was similar. EF augmentation in patients with DCMP was greater than
that in patients with IHD and depended on improvement of segmental contractility.
Conclusion. Significant differences were not found in the action of two P -blockers on global and regional
contractility of the left ventricle in patients with CCF.

Effects of trimetazidine and perindopril on myocardialfunction and metabolism in postmyocardial infarction patients with cardiac failure
Abstract
Aim. To investigate effects of trimetazidine and perindopril on myocardial function and metabolism in
patients with cardiac failure after myocardial infarction.
Material and methods. A comparative study was made of the effects of 6-month therapy with perindopril,
trimetazidine alone and in combination on clinical symptoms, left ventricular function and perfusion,
glucose utilization, excretion and utilization of fatty acids in the zones of postinfarction scar,
periinfarction ischemia and in without infarction changes.
Results. Combined treatment with the ACE inhibitor and trimetazidine produced positive clinical and
functional-metabolic shifts in postmyocardial infarction patients with cardiac failure.
Conclusion. The addition to the program of mild cardiac failure treatment early after myocardial infarction
of an ACE inhibitor and a cytoprotective drug (trimetazidine) promotes more complete clinical
compensation of the patients.

Long-term results of structum® administration (data from a multicenter trial)
Abstract
Aim. To evaluate duration of a clinical response to the drug structum in patients with osteoarthrosis (OA)
of the knee and hip joints as well as structum effects on OA course and that of concomitant diseases.
Material and methods. The duration of a clinical response to structum after the end of the treatment
and its effect on OA and concominant diseases course were studied for 12 months in 9 centers participating
in the study of the drug efficacy and tolerance in patients with gon- and coxarthrosis in an open
multicenter randomized controlled 6-month trial. Out of 555 patients with OA of the knee and hip
joints enrolled in the first study, the examination covered 373 patients: 159 patients of the test group
treated for 6 months with structum and 214 controls. By basic clinical parameters the groups were similar.
Clinical examination was made after structum treatment and 12 months later and included assessment
of the number of exacerbations, hospitalizations, outpatient consultations, days of temporary
disability for OA, pain in the joints while walking and at rest by the visual scale, Leken's functional
index, x-ray pictures of the joints, administration of nonsteroidal anti-inflammatory drugs (NSAID),
exacerbations of concomitant diseases (gastrointestinal diseases, arterial hypertension, ischemic heart
disease).
Results. An overall functional Leken's index in patients with gon- and coxarthrosis given structum 12
months after the treatment did not reach the initial values as well as pain and daily need in NSAID.
Structum effect in patients with knee joint OA persisted for 4.6 months , in hip joint OA - 4.1 months;
in patients with stage /-// the effect lasted longer than in stage III (5.2 and 4.6 months vs 4.17 and
3.24 months, respectively. Even short-term therapy with structum reduced the number of further exacerbations,
hospitalizations and visits to their doctor. 12 months after structum therapy the effect persisted
in 40% patients. Frequency of exacerbations of the concomitant diseases was less than in patients
on continuous NSAID.
Conclusion. Structum is a highly effective drug against OA as it acts long, reduces frequency of exacerbations,
hospitalizations, visits to the doctor, duration of disability, NSAID requirement and improves
the course of some concomitant diseases.

Therapeutic efficiency oflow-intensity laser radiation used in combination with actovegin in indolent gastroduodenal ulcers
Abstract
Aim. To validate use of intravenous laser blood irradiation ([LBI) combined with actovegin adminis-
tration in indolent gastroduodenal ulcers.
Material and methods. Modern endoscopic, morphological, device, biochemical techniques and radi-
oimmunoassay were used in examination of 92 patients with indolent gastroduodenal ulcers aged 24 to
69 years. ILBf plus actovegin was given in failure of standard medicinal therapy.
Results. ILBI plus actovegin combination produced marked analgetic, anti-inflammatory and detoxi-
cation effects. Favourable trends were observed in the composition of gastric mucus, detoxication, re-
parative and metabolic processes in the gastroduodenal mucosa, neurohumoral regulation.
Conclusion. Combination of ILBI with actovegin proved highly effective in indolent gastroduodenal
ulcers
