Aim. To study joint pathology in patients with Behcet's disease (BD). Material and methods. The trial enrolled 183 BD patients (128 males, 54 females, mean age 32,0 ± 9.8 years, age at BD onset 22.3 ± 9.9 years, mean duration of the disease 9.5 ± 8.9 years. Results. Joint pathology occurred in 74.8% patients, in 10.9% patients the disease onset was associated with arthritis and arthralgia. 3 patients had aceptic necrosis of the head of the femur and knee joint, 3 patients - bone marrow infarction. Knee joints were affected more frequently than others (up to 40%) followed by ankle, elbow and radiocarpal and other joints. In BD duration about 10 years, none of the examinees had stage III of sacroiliitis, stage II was diagnosed only in 4 patients, stage I (suspicion) - in 9. Significant sacroillitis was not registered, bilateral sacroillitis occurred only in 9 patients. HLA B5 was detected in 61% patients while HLA B27 - only in 9.7%. Conclusion. The articular syndrome was seen in 3/4 of BD patients. Arthritis runs a recurrent course. Erosive arthritis, sacroillitis and combination of the latter with HLA B27 are rare.

Болезнь бехчета и поражение суставов. - Цель исследования. Оценить суставную патологию у пациентов с болезнью Бехчета (ББ). Материалы и методы. В исследование включены 183 больных, 128 мужчин и 54 женщины. Средний возраст на момент обследования составил 32,0 ± 9,8года. Возраст больных в начале болезни - 22,3 ± 9,9 года, средняя продолжительность болезни - 9,5 ± 8,9 года. Результаты. Суставная патология наблюдалась у 74,8% больных; у 10,9% заболевание дебютировало именно с артрита или артралгий. У трех больных диагностирован асептический некроз головки бедренной кости (2) и коленного сустава (1); еще у трех больных при МРТ-исследовании выявлен инфаркт костного мозга. Чаще всего поражались коленные суставы (до 40%), затем голеностопные, локтевые, лучезапястные и др. При давности ББ почти 10 лет ни у одного из обследованных не было III стадии сакроилиита, II стадия диагностирована всего у четырех больных, а I стадия (подозрение на сакроилиит) - у девяти. Диагноз достоверного сакроилиита не был поставлен ни в одном случае; двусторонний сакроилиит (I стадия) имел место только у девяти больных. HLA В5 обнаружен почти у 61% больных, тогда как HLA В27 - всего у 9,7%, что практически не отличается от популяционного контроля. Заключение. Суставной синдром встречается у 3/4 пациентов с ББ. Артрит имеет рецидивирующее течение; это, как правило, недеформирующий моно- или олигоартрит, симметричный или асимметричный. Эрозивный артрит встречается редко. Низкая частота сакроилиита (не выше I-II стадии по Келгрену) и редкое сочетание последнего с HLA В27 не позволяют рассматривать ББ в рамках спондилоартрита.×
