Evaluation of coronary artery lesion in men with osteopenic syndrome and coronary artery disease

Оценка поражения коронарных артерий у мужчин с остеопеническим синдромом и ишемической болезнью сердца. - Резюме. Цель исследования. Изучение взаимосвязи коронарного атеросклероза (АС), кальцификации коронарных артерий (КА) и показателей минеральной плотности костной ткани (МПКТ) у мужчин с ишемической болезнью сердца (ИБС). Материалы и методы. В исследование включили 74 мужчин в возрасте 60,1 (55; 70) года с подтвержденной ИБС. Проводили рентгеноконтрастную селективную коронарографию, оценку кальциевого индекса (КИ) КА, определяли МПКТ методом остеоденситометрии. Тяжесть поражения КА оценивали с использованием шкалы SYNTAX. Пациентов разделили на 3 группы в зависимости от значения Т-критерия: больные с остеопорозом (ОП) - 1-я группа (n=23), с остеопенией - 2-я группа (n=30), с нормальными показателями МПКТ - 3-я группа (n=21). Результаты. Остеопенический синдром - ОС (ОП и остеопения) определили у 71,6% больных с ангиографически подтвержденной ИБС. Однососудистое поражение коронарного русла (КР) в группе с нормальной МПКТ регистрировали чаще, чем в группе с ОП (р=0,023). Тяжелое поражение КР более характерно для пациентов с ОП (р=0,029). Выявлены статистически значимые различия между группами с остеопенией и нормальной МПКТ в отношении тяжелой степени поражения КР (р=0,042). Различия по суммарному КИ отмечены между 1-й и 2-й группами (р=0,0122). КИ статистически значимо коррелировали с тяжестью поражения КА по шкале SYNTAX (r=0,53; p=0,002). Заключение. Тяжелое поражение КР у мужчин коррелирует с коронарным кальцинозом и ассоциируется со снижением МПКТ, что позволяет предполагать общность патогенетических звеньев АС и ОС.About the authors
