Relationship of subclinical atherosclerosis indicators to exercise tolerance in patients with coronary heart disease

Зависимость показателей субклинического атеросклероза от толерантности к физической нагрузке у больных ишемической болезнью сердца. - Резюме. Цель исследования. Изучение распространенности ряда показателей субклинического атеросклероза у пациентов с хроническими формами ишемической болезни сердца (ИБС) и характера их зависимости от функционального состояния и объема мышечной массы. Материалы и методы. Обследовали 20 пациентов с хроническими формами ИБС. О жесткости сосудистой стенки судили по скорости распространения пульсовой волны (СРПВ) по сосудам эластического типа с вычислением каротидно-феморального индекса (КФИ). Функцию эндотелия оценивали по зависимой от эндотелия вазодилатации (ЗЭВД) в пробе с реактивной гиперемией. Для оценки толщины комплекса интима-медиа (ТИМ) сонных артерий использовали ультразвуковое сканирование общей сонной артерии. Всем пациентам проводили велоэргометрию; у всех определяли толерантность к физическим нагрузкам (ТФН) по количеству потребляемого кислорода, при вычислении метаболического эквивалента (МЕТ); оценивали объем мышечной массы, определяли процент активной клеточной массы (%АКМ) и процент безжировой массы (%БЖМ) от общей массы тела; оценивали уровень общего холестерина (ХС), ХС липопротеидов высокой плотности, низкой плотности, триглицеридов. Контрольную группу составили 20 сопоставимых лиц без ИБС. Результаты. Прогностически неблагоприятное увеличение КФИ >12 м/с выявлено у 20% пациентов с ИБС и у 10% контрольной группы, вазомоторная дисфункция эндотелия (ЗЭВД <10%) - у 65 и 50%, увеличение ТИМ более 0,9 мм - у 55 и 15% соответственно. У большинства пациентов с патологической жесткостью сосудов и вазомоторной дисфункцией эндотелия констатирована низкая ТФН. У больных ИБС с низкой ТФН выявлены достоверные корреляции КФИ с %АКМ и %БЖМ, ЗЭВД и %АКМ и %БЖМ, КФИ и ЗЭВД. У пациентов контрольной группы с низкой ТФН также выявлены корреляции ЗЭВД и %АКМ и %БЖМ. В обеих группах выявлена корреляция %АКМ и %БЖМ с МЕТ. Заключение. ЗЭВД и СРПВ отражают снижение уровня физической активности и дезадаптации функционального состояния пациента в большей степени, чем уменьшение выраженности субклинического атеросклероза.About the authors
