Somatogenic and somatogenically provoked psychoses in blood diseases

Abstract

Aim. To detect risk factors (RF) to develop somatogenic psychoses (SP) in blood diseases. Material and methods. A total of 107 SP patients were examined with the disease corresponding to diagnostic points F05 or F06 (IDC-10). Results. The following RF were identified: cytostatic drugs with or without glucocorticosteroids, glucocorticosteroids alone, interferon-alpha, viral encephalitis, neuroleukemia. Single obligatory specific factors which bring manifestation of certain psychoses were not identified. SP in each of the considered hematological malignancies are related with various factors. Conclusion. The findings suggest polyfactor etiology of SP including some schemes of chemotherapy, some type of blood disease, specific premorbid features and mental disease burden. We suggest that the clinical picture, dynamics and prognosis of SP in blood diseases may be caused by a profile of factors typical for each disease.

Соматогенные и соматогеннопровоцированные психозы при заболеваниях системы крови. факторы риска развития самотогенных психозов при заболеваниях системы крови. - Цель исследования. Выявить факторы риска развития соматогенных психозов (СП) при заболеваниях системы крови. Материалы и методы. Были клинически обследованы 107 пациентов, у которых наблюдались СП, соответствовавшие на момент развития диагностическим рубрикам F05 или F06 МКБ-10. Результаты и обсуждение. Выделены следующие факторы риска развития СП у больных с заболеваниями системы крови: применение цитостатических препаратов без глюкокортикоидов (ГК), цитостатических препаратов с ГК, α-интерферона (ИФ-α), вирусные энцефалиты, нейролейкемия, монотерапия ГК. В ходе проведенного исследования не было выявлено единичных облигатных специфических факторов, обусловливающих манифестацию определенных психозов. Напротив, СП при каждом из рассматриваемых онкогематологических заболеваний обнаруживают связь с множеством различных факторов. Заключение. Полученные данные позволяют предполагать полифакторную этиологию СП, т. е. сочетание двух и более факторов, включая определенные схемы химиотерапии, конкретный тип заболевания системы крови, преморбидные особенности и психические расстройства в анамнезе. На основании полученных данных можно сформулировать предположение, что определяющее значение для клинической картины, динамики и прогноза СП при заболеваниях системы крови приобретает "профиль" факторов, типичных для каждого заболевания.
