АНТИТЕЛА К ЦИКЛИЧЕСКОМУ ЦИТРУЛЛИНИРОВАННОМУ ПЕПТИДУПРИ РЕВМАТОИДНОМ АРТРИТЕ

Аннотация

Цель исследования. Изучить диагностическое значение антител к циклическому цитруллинированному
пептиду (аЦЦП) при ревматоидном артрите (РА).
Материалы и методы. Обследовали 85 больных РА, в том числе 48 человек с "ранним" РА -длительностью
менее 8 мес. В группу сравнения вошли 35 больных недифференцир'овнным артритом
(НДА), в контрольную группу - 8 здоровых (доноров). Концентрацию аЦЦП, IgM ревматоидного
фактора (РФ) и IgA РФ определяли иммуноферментным методом (ИФМ).
Результаты. Уровень аЦЦП у больных РА (76,3 ± 43,8 ЕД/мл; медиана 100,0 ЕД/мл) был
достоверно выше, чем у больных НДА (25,1 ± 43,9 ЕД/мл; медиана 0,8 ЕД/мл; р < 0,05) и у
доноров (0,38 ± 0,36 ЕД/мл; медиана 0,2 ЕД/мл; р < 0,05). Отмечена выраженная связь между
результатами определения аЦЦП и уровнем IgM РФ (¦/ = 15,4; р = 0,001) и IgA РФ
(Х2 - 10,3; р = 0,001). Чувствительность (82 %) и специфичность (90 %) аЦЦП в диагно-
стике РА превышали данные показатели IgM РФ (78 и 86 % соответственно) и IgA РФ (72 и
83 % соответственно). Одновременное тестирование аЦЦП, IgM РФ и IgA РФ приводило к
увеличению специфичности до 93 %. аЦЦП выявлялись у 50 % больных, серонегативных по
IgM РФ, и у 62 % пациентов, серонегативных по IgA РФ. Обнаружение аЦЦП тесно ассоциировалось
с "ранним"PA (у/ = 30,8;р = 0,0001).
Заключение. Определение аЦЦП с помощью ИФМ является чувствительным и специфичным
серологическим тестом для ранней диагностики РА.
