The study included 800 patients with an established diagnosis of IBD. Grounds for diagnostic reclassification were analyzed. The primary endpoint was the cumulative frequency of diagnostic reclassification. Secondary endpoints included patterns of reclassification in patients initially diagnosed with unclassified IBD and ulcerative colitis (UC), median time to diagnostic revision, the clinical and morphological grounds for reclassification.</p> <p><bold>Results.</bold> During follow-up, diagnostic reclassification occurred in 17.6% of cases (<italic>n</italic>=141). Among reclassified patients, the initial diagnosis was IBDU in 16.3% (<italic>n</italic>=23) and UC in 83.7% (<italic>n</italic>=118). In 93.6% of cases (<italic>n</italic>=132), reclassification resulted in Crohn’s disease (CD) (95% CI 88.2–97.0); in 9 patients, the final diagnosis was unknown due to missing data. The median time from the initial diagnosis to reclassification was 42.0 months [16.0; 99.0]. The most common reason for diagnostic revision was the identification of segmental involvement: 50.0% (7/14) among patients initially classified as IBDU and 63.6% (75/118) among those initially diagnosed with UC (<italic>p</italic>=0.386). In the presence of a stricture, the median time to reclassification was 96 months [37.0; 186.0].</p> <p><bold>Conclusion.</bold> Diagnostic reclassification in patients with IBD is a prolonged and clinically meaningful process. The findings support the possibility of diagnostic reassessment in patients initially diagnosed with UC or IBDU. In clinical practice, when establishing an IBD diagnosis – particularly UC – it is important to consider the potential for reclassification, and when indications for revision are present, to document the fact of reclassification.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Цель.</bold> Оценить кумулятивную частоту и варианты реклассификации диагноза при воспалительных заболеваниях кишечника (ВЗК).</p> <p><bold>Материалы и методы.</bold> Одноцентровое ретроспективное продольное когортное исследование проходило на базе отделения гастроэнтерологии Клиники им. Петра Великого ФГБОУ ВО «СЗГМУ им. И.И. Мечникова». Размер выборки – 800 пациентов с установленным диагнозом ВЗК, наблюдавшихся в 2020–2025 гг. Первичная конечная точка: кумулятивная частота реклассификации диагноза при ВЗК. Вторичные конечные точки: распределение направлений реклассификации диагноза недифференцированного ВЗК и язвенного колита (ЯК), медиана времени от момента установления первичного диагноза до его пересмотра, частота и структура причин реклассификации диагноза.</p> <p><bold>Результаты.</bold> В ходе наблюдения за пациентами в 17,6% случаев (<italic>n</italic>=141) имела место реклассификация диагноза. Исходно диагноз недифференцированного ВЗК установлен у 16,3% (<italic>n</italic>=23) пациентов, диагноз ЯК – у 83,7% (<italic>n</italic>=118) пациентов. В 93,6% случаев (<italic>n</italic>=132) исходом реклассификации явилась болезнь Крона (95% доверительный интервал 88,2–97,0); у 9 пациентов исход неизвестен в связи с отсутствием данных. Медиана времени от момента установления первичного диагноза до его реклассификации составила 42,0 мес [16,0; 99,0]. Наиболее частым основанием для пересмотра явилось выявление сегментарности поражения: 50,0% (7/14) при исходном недифференцированном ВЗК и 63,6% (75/118) – при исходном ЯК (<italic>p</italic>=0,386). При наличии стриктуры медиана срока до реклассификации составила 96 мес [37,0; 186,0].</p> <p><bold>Заключение.</bold> Реклассификация диагноза у пациентов с ВЗК рассматривается как длительный и клинически значимый процесс. Полученные данные подтверждают допустимость «переоценки» диагноза у пациентов с первоначально установленным ЯК или недифференцированным ВЗК.
