Modern therapeutic approaches often focus solely on symptomatic cough suppression, while targeting its pathogenetic mechanisms, including MCP-dependent inflammation, remains understudied. The article examines the innovative antitussive drug Eladis<sup>®</sup>, designed to target chemokine-dependent mechanisms involved in the development of cough sensitization.</p> <p><bold>Aim</bold><bold>.</bold> To conduct a systematic analysis of published data on the mechanisms of action, clinical efficacy, and safety of Eladis<sup>®</sup>, a novel antitussive drug that inhibits MCP-dependent inflammation, based on experimental and clinical trial results (phases I–III).</p> <p><bold>Materials</bold><bold> </bold><bold>and</bold><bold> </bold><bold>methods</bold><bold>. </bold>The review synthesizes experimental data from <italic>in</italic><italic> </italic><italic>vitro</italic> and <italic>in</italic><italic> </italic><italic>vivo</italic> studies (guinea pigs), demonstrating selective suppression of TRPA1-dependent cough via inhibition of Golgi-resident glutamyl cyclase. Clinical trials included: phase I: pharmacokinetics and safety assessment in healthy volunteers (<italic>n</italic>=32); phase II: evaluation of lung function in patients with partially controlled asthma (<italic>n</italic>=250); phase III: randomized placebo-controlled study in acute respiratory viral infections and acute bronchitis (<italic>n</italic>=250).</p> <p><bold>Conclusion</bold><bold>.</bold> Eladis<sup>®</sup> is a promising antitussive drug with a unique mechanism targeting MCP-dependent inflammation. It is effective for acute non-productive cough and offers new therapeutic potential for chronic respiratory diseases. Further research may expand its clinical applications.</p></abstract><trans-abstract xml:lang="ru"><p>Кашель, являясь защитным рефлексом, при патологических состояниях, в частности при острой респираторной вирусной инфекции, остром бронхите, астме, может приобретать избыточный или гиперчувствительный характер, значительно ухудшая качество жизни пациентов. Современные методы лечения часто направлены лишь на симптоматическое подавление кашля, тогда как воздействие на его патогенетические механизмы, включая MCP-зависимое (Monocytic Chemotactic Protein – моноцитарный хемотаксический протеин) воспаление, остается малоизученным. В статье проанализирован инновационный противокашлевый препарат Эладис<sup>®</sup>, разработанный для влияния на хемокин-зависимые звенья формирования кашлевой сенситизации.</p> <p><bold>Цель.</bold> Провести систематический анализ опубликованных экспериментальных и клинических данных о механизмах действия, клинической эффективности и безопасности нового противокашлевого препарата Эладис<sup>®</sup> (N-[2-(1H-имидазол-4-ил)-этил]-6-оксо-дельта-лактам), ингибирующего MCP-зависимое воспаление, на основе результатов экспериментальных и клинических исследований (I–III фазы).</p> <p><bold>Материалы и методы. </bold>В обзоре обобщены экспериментальные данные<italic> </italic><italic>in</italic><italic> </italic><italic>vitro</italic> и <italic>in</italic><italic> </italic><italic>vivo</italic>, продемонстрировавшие селективное подавление TRPA1-зависимого кашля через ингибирование Гольджи-резидентной глутаминилциклазы. Включены клинические исследования: I фаза – оценены фармакокинетика и безопасность у здоровых добровольцев (<italic>n</italic>=32); II фаза – рассмотрено влияние на функцию легких у пациентов с частично контролируемой астмой (<italic>n</italic>=250); III фаза – проведено рандомизированное плацебо-контролируемое исследование при острой респираторной вирусной инфекции и остром бронхите (<italic>n</italic>=250).</p> <p><bold>Заключение.</bold> Эладис<sup>®</sup> представляет собой перспективный противокашлевый препарат с уникальным механизмом действия, направленным на MCP-зависимое воспаление через ингибирование Гольджи-резидентной глутаминилциклазы. Доказана его эффективность в лечении острого непродуктивного кашля у больных острой респираторной инфекцией, выявлен терапевтический потенциал при хронических респираторных заболеваниях.
