The high risk of untimely death in these diseases has been found to be associated with the severity of an immunoinflammatory process that gives rise to severe irreversible damage to vital organs and systems and with the development of a wide spectrum of comorbidities (infections, interstitial lung disease, malignant tumors, osteoporotic fractures, etc.). Among them, diseases of the cardiovascular system, which are most commonly caused by the early development and accelerated progression of atherosclerotic coronary lesions, hold a central position. The paper gives the data available in the recent literature on the impact of antirheumatic therapy (disease-modifying antirheumatic drugs and biological agents) on the cardiovascular system.</p></abstract><trans-abstract xml:lang="ru"><p>Представителями иммуновоспалительных заболеваний являются ревматические болезни, в первую очередь ревматоидный артрит, ювенильный идиопатический артрит, спондилоартриты, псориатический артрит, подагрический артрит, системная красная волчанка и другие системные заболевания соединительной ткани, для которых характерен высокий риск преждевременной смерти больных. Установлено, что высокий риск преждевременной смерти при этих заболеваний связан с «тяжестью» иммуновоспалительного процесса, приводящего к тяжелому необратимому повреждению жизненно важных органов и систем, и с развитием широкого спектра сочетанных состояний (инфекции, интерстициальное заболевание легких, злокачественные новообразования, остеопоретические переломы и др.). Среди них центрально место занимает патология сердечно-сосудистой системы, наиболее часто обусловленная ранним развитием и ускоренным прогрессированием атеросклеротического поражения коронарных сосудов.
