To assess the results of diagnosing and treating Pneumocystis pneumonia (PP) in patients with Hodgkin lymphoma (HL) over 15 years. MATERIALS AND METHODS. In 1999 to 2013, PP occurred in 22 (3%) of 741 HL patients receiving programmed polychemotherapy (PCT). The male/female ratio was 1:1.1; median age was 32 (18-65) years. Advanced stages (IIB-IV) of the disease were seen in 82% of the patients. The diagnosis of PP was established when Pneumocystis (more than 5 cysts in the specimen) was detected in the lavage fluid by indirect immunofluorescence assay. RESULTS. PP developed after 4 or more cycles of PCT. Along with Pneumocystis, all the cases were found to have additional pathogens: herpes virus in 72% and bacteria and fungi in 33%. All the patients received combined antimicrobial therapy using high doses of intravenous trimethoprim-sulfamethoxazole. Ten (45%) patients required mechanical ventilation (MV). The total mortality in PP was 32% (7 patients died); moreover, none of the patients without MV died whereas the mortality among those who had MV was 70% (7 of the 10 patients died). High death rates (80%) were noted among the patients with recurrent and resistant HL. CONCLUSION. PP should be prevented with trimethoprim-sulfamethoxazole in patients with LH during PCT. If respiratory failure and X-ray signs of interstitial pneumonia appear, there is a need for fibrobronchoscopy with bronchoalveolar lavage and comprehensive microbiological testing of lavage fluid.</p></abstract><trans-abstract xml:lang="ru"><p>Резюме. Цель исследования. Оценка результатов диагностики и лечения пневмоцистной пневмонии (ПП) у больных лимфомой Ходжкина (ЛХ) за 15 лет. Материалы и методы. За период с 1999 по 2013 г. ПП возникла у 22 (3%) из 741 больного ЛХ, получавших программную полихимиотерапию (ПХТ). Соотношение мужчин и женщин составило 1:1,1, медиана возраста 32 (18-65 лет) года. У 82% больных имелись развернутые стадии болезни (IIB-IV). Диагноз ПП устанавливали при выявлении в лаважной жидкости пневмоцист методом реакции непрямой иммунофлюоресценции (более 5 пневмоцист в препарате). Результаты. ПП развивалась после 4 курсов ПХТ и более. Наряду с пневмоцистой во всех случаях выявлены дополнительные возбудители: вирус герпеса у 72% больных, бактерии и грибы у 33%. Всем пациентам проводили комбинированную противомикробную терапию с применением триметоприма/сульфаметоксазола в высоких дозах внутривенно. Искусственная вентиляция легких (ИВЛ) потребовалась 10 (45%) пациентам. Общая летальность при ПП составила 32% (умерли 7 пациентов), при этом среди пациентов без ИВЛ не умер ни один, в то время как среди пациентов, находившихся на ИВЛ, летальность составила 70% (умерли 7 из 10 пациентов). Отмечена высокая летальность (80%) среди больных с рецидивами и резистентным течением ЛХ. Заключение. Больным ЛХ в период ПХТ необходима профилактика ПП препаратом триметопримом/сульфаметоксазолом. При появлении дыхательной недостаточности и рентгенологических признаков интерстициальной пневмонии необходимо выполнение фибробронхоскопии с бронхоальвеолярным лаважем и комплексным микробиологическим исследованием лаважной жидкости.</p></trans-abstract><kwd-group xml:lang="en"><kwd>Pneumocystis pneumonia</kwd><kwd>Hodgkin lymphoma</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>пневмоцистная пневмония</kwd><kwd>лимфома Ходжкина</kwd></kwd-group></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Боровицкий В.С.
